The purpose of this conference is to promote open communication about your care and services and to answer questions, personalize your service plan, as well as address any issues or concerns.

The scheduled conference should only take 15 - 30 minutes. Care conferences are based on a personal services assessment which is a functional resident assessment tool that identifies your needs and preferences. Our system also includes an individualized personal service plan to provide a blend of personal care and support services, customized to your needs.
This personal service assessment, will determine any cost of services per month. We will use this tool to evaluate your needs at move-in, upon change of condition and at regularly scheduled intervals, or more frequently based on state regulations.

We value our partnership with families and we encourage participation in these important conferences, as agreed to by the resident, whether in person, or by phone. Your valuable feedback and input will contribute greatly to the care and services your loved one receives. During these care conferences, we invite you to review the assessments and encourage you to speak with us, should there be any questions.

Our ongoing care conferences provide valuable opportunities for feedback on the services received and an opportunity to discuss any changes that may have occurred.
